Forms & Applications
This is the process to apply for appointed positions with Fraternity and Sorority Life. Electronic applications will be available in October 2013. Please note that positions apply to all councils (IFC, MGC, NPHC or PHC), unless otherwise noted in the application.

Philanthropy Registration Form
Register your Fall 2013 philanthropy by April 15, 2013! Forms are due to the Center for Student Involvement & Leadership (145 IMU) by 4pm. Click HERE to get the 2013 Fall Registration Form!
Philanthropy Evaluation Form
Within 30 days of the completion of your philanthropy, each chapter must complete the Philanthropy Evaluation Form. Along with this evaluation, the chapter must submit a letter from your charity OR a copy of the check written confirming the receipt of your payment to the philanthropy.
Social Registration Forms

Educational Programming Form
Two (2) "in chapter" educational programs are required each semester per chapter. Please complete the form below and submit to IFC/PHC within 10 days of the completition of the program.
NPHC & MGC Membership Intake Form
Any chapters wanting to particpate in Membership Intake, must complete the Membership Intake Form. This form is due to Kelly Jo or Leslie by October 15th or March 1st of that respective semester.
